ABSTRACT

Objective@#The severity of obstructive sleep apnea hypopnea syndrome (OSAHS) has a tendency to increase with age. The purpose of this study was to explore whether oral appliance (OA) treatment can block this age-related change.@*Methods@#This study was a retrospective study. Fifteen patients (12 males,3 females) of OSAHS treated with OA were selected as treatment group,with an average age of (47.44±10.00) years and initial body mass index (BMI) of (26.31±3.33) kg/m2. The follow-up length was 54 [22, 100] months. Nineteen patients (13 males,6 females) with untreated OSAHS served as controls, with an average age of (45.00±9.26) years and initial BMI of (25.53±2.58) kg/m2,and the follow-up length was 35 [26,63] months. There were no significant differences in terms of gender,age,initial BMI, apnea hypopnea index(AHI), and follow-up length between the two groups. Polysomnography(PSG) data for the two groups were compared to observe the sleep respiratory function changes as aging by Wilcoxon test.@*Results@#There was no significant difference in BMI of the treatment group and the control group at the time of follow-up, with BMI of treatment group from (26.31±3.33) kg/m2 to (25.67±3.65) kg/m2,Z=-1.223,P=0.221; and BMI of control group from (25.53±2.58) kg/m2 to (25.12±2.72) kg/m2,Z=-1.193,P=0.233. There was no significant difference in the change of AHI within the treatment group, from 26.20 [11.50, 52.98]/h to 23.10 [16.00, 45.00]/h, Z=-0.284, P=0.776; AHI in the control group was higher than that at the first visit, and the AHI increased from 15.00 [10.72, 28.90]/h to 31.10 [13.00, 41.80]/h, Z=-3.481, P<0.001. The longest apnea duration was not statistically different in the treatment group, from 60.00 [56.40, 74.00] s to 63.00 [52.00, 77.00] s, Z=-0.345, P=0.730; the longest apnea duration in the control group increased from 42.00 [34.00, 56.70] s to 46.00 [37.00,62.00] s,Z=-2.274,P=0.023. There was no significant difference in the lowest blood oxygen saturation of the treatment group and the control group, with the treatment group from 72.47%±12.69% to 72.73%±17.59%, Z=-0.597, P=0.550; and the control group from 78.21%±9.30% to 76.42%±12.17%, Z=-0.153, P=0.879.@*Conclusion@#Symptoms of sleep apnea in OSAHS patients tend to increase with age,and oral appliance treatment may have the effect of slowing down this age-related worsening effect.

ABSTRACT

Glucaric acid (GA), a top value-added chemical from biomass, has been widely used for prevention and control of diseases and the production of polymer materials. In GA biosynthesis pathway, the conversion of inositol to glucuronic acid that catalyzed by myo-inositol oxygenase is the limiting step. It is necessary to improve MIOX activity. In the present study, we constructed a high-throughput screening system through combing the concentration of GA with the green fluorescent protein fluorescence intensity. By applying this screening system, three positive variants (K59V/R60A, R171S and D276A) screened from the mutant library. In comparison, the recombinant strain Escherichia coli BL21(DE3)/MU-R171S accumulated more GA, 136.5% of that of the parent strain.

ABSTRACT

Objective@#To evaluate the effect of sandblasting on bending strength and subcritical crack growth (SCG) under cyclic loading of yttria-stabilized tetragonal zirconia polycrystal (Y-TZP) ceramics.@*Methods@#After being polished, sixty bar-shaped specimens of Y-TZP (Wieland zirconia ceramics) were assigned to two groups (n=30) according to the random number table, the sandblasting group (SG) which was treated with sandblasting and the control group (CG) which remained untouched. In each group, half of the specimens (n=15) were subjected to bending strength test, and the results were examined by Weibull statistics and analyzed with ANOVA. The other 15 specimens in each group were subjected to fatigue tests. The results were examined by Weibull statistics and subcritical crack propagation rates were calculated.@*Results@#The bending strengths of SG and CG were (1 291±133) and (1 140±124) MPa (F=10.117, P=0.004), and the Weibull modules of the two groups were 11.06 and 10.64 respectively. The crack growth rate of SCG of SG was lower than that of CG under the same cyclic loading.@*Conclusions@#Proper sandblasting on Y-TZP ceramic can increase its bending strength and resistance to SCG.

ABSTRACT

Objective To explore the related factors with skin flap necrosis,we concluded the cases of patients with skin defects after free flap plantation.Methods From 2001 to 2016,188 cases about 20 influencing factors were analyzed (The characteristics of patients:age,sex,smoke,diabetes,high blood pressure;Preoperative factors:injured sections,injured causes,preoperative wound infection,preoperative wound osteomyelitis,the time from injury to operation;Intraoperative factors:operator,operation time,anesthesia time,intraoperative rehydration fluids,the way of vascular anastomosis,the number of venous anastomosis,the area of flap;Postoperative factors:flap hematoma,flap infection,vascular crisis) and multivariate logistic regression analysis was used to analyze the relationship between these risk factors and flap necrosis.Results All 188 cases were treated with free anterolateral thigh flap to repair soft tissue defect and it was revealed that the 174 cases were successful (92.55%) and 23 cases were occured vascular crisis (12.23%),8 cases were arterial crisis,11 cases were vein crisis,4 cases were ateriovenous crisis.After the treatment,the rescue was successful in 5 cases (38.46%).After the analysis we made the conclusion that the number of venous anastomoses,flap hematoma and vascular crisis were related with the skin flap necrosis.Conclusion The number of venous anastomose (≥2) will increase blood return to make the flap easier to survive.Intraoperative stanching and drainage tube placement work will reduce the skin flap hematoma as a result of reducing the skin flap necrosis.Artery and venous crisis handled in time,can enhance the survival rate of flap.

ABSTRACT

Objective:To investigate the long-term efficacy of oral appliance treating of obstructive sleep apnea and hypopnea syndrome (OSAHS) in different periods of treatment.Methods:A total of 55 patients were included in the study.Patients were all diagnosed with OSAHS by overnight polysomnography and all received oral appliance (OA) as the therapy.The OA positioned the mandible at 60%-70% of the maximal mandible advancement position and created a 4-5 mm incisor separation.The patients were instructed to wear the appliance during sleep,6-8 hours per day,for 5-7 days per week.They were divided into four groups by the period of treatment,including less than 1 year group;1-2 years group;2-6 years group and 6-9 years group.The polysomnographic study was used to investigate the efficacy of the four groups.The outcome measures included the score on the apnea-hypopnea index (AHI),the longest apnea time and the lowest oxygen saturation (LSaO2) levels during an overnight sleep.Results:The AHI decreased significantly in all the four groups.The less than 1 year group decreased from 24.50 (14.65,54.05) to 7.40 (2.12,10.00) events/h (P < 0.001);The 1-2 years group decreased from 19.50 (12.15,39.23) to 1.80 (0.70,6.58) events/h (P =0.001);The 2-6 years group decreased from 25.00 (11.41,42.60) to 4.50 (1.35,7.90) events/h (P =0.001);The 6-9 years group decreased from 26.2 (16.95,47.45) to 4.00 (1.90,26.70) events/h (P =0.043).The longest apnea decreased significantly in less than 1 year group,1-2 years group and 2-6 years group.The longest apnea decreased from 57.00 (37.70,61.50) to 25.00 (15.90,33.50)seconds (P<0.001) in the less than 1 year group,from 41.00 (25.50,62.26) to 13.10 (0.00,22.10) seconds (P =0.001) in the 1-2 year group and from 42.50 (30.35,58.15) to 15.60 (0.00,28.10) seconds (P =0.003) in the 2-6 year group.The LSaO2 levels increased significantly in the less than 1 year group and 2-6 years group.The LSaO2 levels rose significantly in the less than 1 year group,from 74.18% ±7.96% to 84.06% ±7.67% (P =0.001),and in the 2-6 years group,from 76.71% ± 10.98% to 84.06% ±4.64% (P =0.006),The LSaO2 levels did not increase significantly in the 1-2 years and 6-9 years groups.Kruskal-Wallis test showed that there were no statistically significant differences in the AHI,longest apnea time and LSaO2 in 4 the groups.Conclusion:The oral appliance is an effective therapy for patients with OSAHS in the long-term treatment.However,it's recommended to make appointments with patients as a follow-up supervision whether there is any efficacy decrease.And the oral appliance should be replaced if necessary.

ABSTRACT

Glucaric acid, a high value-added organic acid, is widely used in food, pharmaceutical and chemical industries. For microbial production of glucaric acid in Saccharomyces cerevisiae, we constructed a synthetic glucaric acid biosynthetic pathway by coexpressing the genes encoding myo-inositol oxygenase from mice and uronate dehydrogenase from Pseudomonas putida. Moreover, myo-inositol-1-phosphate synthase was identified as a rate-limiting enzyme in glucaric acid pathway and was upregulated, resulting in the production of glucaric acid of (107.51±10.87) mg/L, a 2.8-fold increase compared to the parent strain. Then, by repressing the activity of phosphofructokinase, the concentration of glucaric acid further increased to (230.22±10.75) mg/L. The strategy could be further used to construct cell factories for glucaric acid production.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the F(-) releasing and recharging properties of six dental restorative materials.</p><p><b>METHODS</b>Disc specimens 10 mm in diameter and 1 mm in thickness were prepared from 6 different dental restorative materials: Fuji VII [glass ionomer cement (GIC)], Fuji II LC (light-curing, GIC), Beautifil (giomer), Compoglass F (compomer), Charisma (composite) and Experimental I (a fluoride releasing composite), with 10 discs for each material. Specimens were dipped in 5 ml deionized water and F(-) release was detected using a fluoride-specific ion electrode every day from 1-7 days and every three days from 8-28 days. On day 28, specimens were exposed to a fluoride foam for 4 min and then dipped in water, then the F(-) concentration was detected every day for a week. The specimens were exposed to fluoride foam and fluoride release was detected repeatedly for three times.</p><p><b>RESULTS</b>All materials presented the highest F(-) release on the first day after dipping and the F(-) release sharply decreased after 24 h and slowly decreased after 3 days. On the first day, Fuji VII presented the highest F(-) release[(99.68±15.21) µg×cm(-2)×d(-1)], followed by Fuji II LC [(37.12±1.67) µg×cm(-2)×d(-1)], Experimental I [(22.93±1.53) µg×cm(-2)×d(-1)], Compoglass F[(15.28±0.70) µg×cm(-2)×d(-1)], Beautifil[(2.40±0.52) µg×cm(-2)×d(-1)] and Charisma[(0.11±0.02) µg×cm(-2)×d(-1)]. Within 28 days of dipping, both Fuji VII and Fuji II LC released more F(-) than other materials did(P < 0.05), followed by Compoglass F and Experimental I, and Beautifil and Charisma released the least F(-)(P < 0.01). After F(-) uptake, all materials released the highest F- on the first day and presented sharply decrease after 24 h and slowly decrease after 2 days. On the first day after F(-) uptake, Fuji VII presented the highest F(-) release (>40 µg×cm(-2) ×d(-1)), much more than other materials(P < 0.01), followed by Fuji II LC(>25 µg×cm(-2) ×d(-1)). Beautifil, Compoglass F and Experimental I were close in F(-) release value(15- 20 µg × cm(-2) × d(-1)), and Charisma showed the lowest F(-) release.</p><p><b>CONCLUSIONS</b>Two glass ionomers were shown to have highest capacity in F(-) release and uptake, followed by compomer and fluoride releasing composite. Composite demonstrated the lowest F(-) release and uptake and Giomer was comparable to composite in F(-) release and to compomer and fluoride releasing composite in F(-) uptake.</p>

ABSTRACT

Objective:To evaluate the reliability of flexural strength and subcritical crack growth(SCG)of WL and HT zirconia ce-ramics under cyclic loading.Methods:Bar-shaped speciments of WL(n =30)and HT(n =30)were fabricated and loaded in three-point bending and cyclically fatigued.The flexural strength and the number of cycles to fracture were obtained from three-point flexural test and cyclic fatigue test respectively.Data were examined using Weibull statistics.Pairs law was used to analysize the subcritical crack growth(SCG),the behavior of which was described in terms of crack velocity as a function of maximum stress intensity factor (KImax ).Results:The Weibull module(m)of WL and HT was 10.64 and 12.04,the stress corrosion cracking susceptive indexes(n) 23.71 and 32.87,the SCG 90% and 76.7%,the KImax 49.6% and 50.9% of its maximum stress intensity factor(KIC ),respectively. Conclusion:WL and HT produce pronounced SCG.The minimum fatigue crack growth rate of HT is lower than that of WL(νHT <νWL ).

ABSTRACT

Objective To analysis the clinical diagnosis and treatment of the scaphoid and lunate bone fracture with (or) wrist dislocation.Methods A retrospective analysis of the clinical data included 77 cases,which got admitted according to the inclusion and exclusion criteria and followed up completely from 2005 to 2013.In this group,there were 64 males and 13 females whose age arranged from 15 to 73 with a mean age of 31.9.There were 41 cases of simple fracture,32 cases of bone fracture with dislocation and 4 cases of simple dislocation.Treatment of cases was divided into the conservative treatment group (9 cases) and the surgery group (68 cases).The surgery group was further divided into four subgroups according to surgical method (i.e.compression screw fixation,Kirschner wire fixation,proximal row of carpal resection,and lunate excision head shaped bone transposition).Clinical outcome was evaluated based on modified Mayo wrist score and X-ray.Results All of 77 patients were followed up completely.Follow-up time arranged from 6 months to 8 years,with an average follow-up time of 3 years and 5 months.9 cases were treated conservatively.In the surgical treatment group,there were 36 cases of compression screw fixation,34 cases of Kirschner wire fixation,1 case of proximal row of carpal resection and 1 case of the lunate bone resection heads transposition.The average healing time of compression screw group was 6.1 weeks and the Kirschner group was 7 weeks.According to the modified Mayo wrist score,excellent:51 cases (66.2%),good:11 cases (14.3%),fair:9 cases,and poor:6 cases.The rate of excellent and good was 80.5%.The cure rate of compression screw treatment was 97%,and 85% of Kirschner.The difference of cure rate was statistically significant.After surgery,there were four cases that got scaphoid malunion.The degree of deformity was not serious with only one case happened in the dominant hand,and the wrist pain got better after symptomatic treatment.1 case of scaphoid fracture with dislocation had the problem of wrist instability.18 weeks later,with ligament tightening surgery,postoperative Mayo score went back to excellent.Conclusion Lateral X-ray film of wrist is a preferred method for the examination of scaphoid and lunate fractures with (or) dislocation.Since it's easy to misdiagnose,films of special position are needed to be added.Surgery is preferred for treatment,and the selecting of surgical methods are based on the situation of fracture and dislocation.The advantages of screw fixation are obvious.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the efficacy of flowable composite resin(FCR) as stress-absorbing liners in Class I cavity restorations in vitro.</p><p><b>METHODS</b>Thirty Class I cavities of 4 mm in diameter and 2 mm in depth were prepared in polycarbonate (PC) plates and divided into three groups, ten each. After application of an adhesive, cavities in each group were restored using one of the following methods: A: restored with Charisma without any lining of FCR; B: lined with Revolution Formula 2 twice before restoration with Charisma; C: lined with Teric Flow twice before restoration with Charisma. All cavities were observed under a photoelastic microscope and photoelastic images were recorded at 3 min and 24 h after curing and the shrinkage stresses on the cavity wall were calculated. The polymerization shrinkage(v%) of the three composite resins was measured using bonded discs method and their elastic moduli were measured according to ISO standard.</p><p><b>RESULTS</b>The shrinkage stresses at 3 min and 24 h of the three methods were as follows,A: (4.93 ± 0.28), (5.87 ± 0.40) MPa, B: (4.90 ± 0.30), (5.84 ± 0.33) MPa, and C: (4.76 ± 0.28),(5.83 ± 0.37) MPa.No significant difference was found in results among different groups. The polymerization shrinkage(v%) in group A,B, and C were (2.63 ± 0.04)%, (4.56 ± 0.06)%, and (3.98 ± 0.02)%. The elastic modulus in group A, B, and C were (9.59 ± 0.65), (4.25 ± 0.51), and (5.41 ± 0.79) GPa.</p><p><b>CONCLUSIONS</b>Under present study condition, using a FCR as stress-absorbing liner under composite resin restoration does not significantly decrease the polymerization shrinkage stresses at the cavity wall.</p>

ABSTRACT

Objective To introduce our experiences of reconstruct the first web space using flaps.Methods One hundred and twelve patients sustained contracture of the first, summarized the indications of different flaps in coverage of the first webspace and their prognosis of these flaps were retrospectively reviewed.Results The local flaps, such as double Z-plasty or Z-plasty, were suitable to cover minor contracture of the first webspace; the dorsal flap arising from the index finger was suitable for the coverage of the medium contracture, and the posterior interosseous artery flap or the reverse antebrachial island flap based on the dorsal branch of the anterior interosseous artery suitable for the severe cases. Conclusion The usage of flaps is important to resume the cosmetic shape and function of the first webspace after release of the contracture of the first webspace.

ABSTRACT

Objective To introduce the clinical application of the extended anterolateral leg flap,which is based on the superficial peroneal artery and lateral supramalleolar artery. Methods Through anatomic study on the blood supplies of the skin over the anterolateral portion of the leg.The combined superficial peroneal artery and lateral supramalleolar artery could extend the size of the traditional anterolateral leg flap.In addition,the point of pivot of the extended anterolateral leg flap could locate at the level of the lateral malleolus,which decreases the distance between the flap and recipient site.Clinically,we have used the extended anterolateral leg flaps in 60 patients to cover skin defect over the feet.The flap ranged from 16 cm×10 cm(pedicle length 8.0 cm)to 26 cm×7 cm(pedicle length 6.0 cm). Results In the series, the flap survived in 53 patients(88.3%)unevently,and partial necrosis occurred in 7 patients(11.7%).Conclusion Based on the combined superficial peroneal artery and lateral supramalleolar artery,the size of the traditional anterolateral leg flap could be inceased,which enlarges its value on the coverage of skin defects over the feet.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the clinical application and discuss the operative indication of the reverse dorsal metacarpal flap and its compound flap on the skin defects of hand.</p><p><b>METHODS</b>From 1990 to 2003, we applied the reverse dorsal metacarpal flap and its compound flap to repair soft tissue defects of fingers in 122 cases, which included 90 cases of the reverse metacarpal flap and 32 cases of its compound flaps with tendon grafts, nerve grafts or bone grafts. Based on the follow-up observations, we analyzed the indications of the reverse metacarpal flap and its compound flaps, the postoperative contours, flap colors and textures in comparison to contralateral fingers retrospectively.</p><p><b>RESULTS</b>In the series of 122 cases, flaps survived and the donor site defects were closed directly. The follow-up period ranged from 1-12 years. The postoperative contours, colors and textures of the flaps and its compound flaps were similar to those of normal fingers, although linear scar remained. According to standards of sense recovery (British Medical Research Council, BMRC), the sense function of the flaps resumed S3 after operation for 1 year. In 10 cases with the tendon defects treated by the flap with tendon grafts, function of flexion-extension of fingers resumed 50%-75% in comparison to the contralateral fingers using the method of measurement of total active motion. In 7 cases with the phalangeal nonunion or bone defects treated by the flap with bone grafts, union occurred after operation for 3 months.</p><p><b>CONCLUSIONS</b>To soft tissue defects on fingers with bone or tendon exposure, the reverse metacarpal flap and its compound flap are a better choice for repairing. The range of repairing is up to the distal interphalangeal joint of fingers. The second dorsal metacarpal artery is more consistent and larger as the choice of vascular pedicle, in comparison with other dorsal metacarpal arteries. Postoperative flap color and texture are similar to normal fingers.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the clinical application of capitate transposition with a pedicle in 30 cases of Kienbock's disease.</p><p><b>METHODS</b>The external diameter and curvature of the capitate head and proximal facet of the lunate were observed and measured in 100 specimens. The vascularity of the capitate was also investigated. Capitate transposition with a vascular pedicle was designed to treat 30 patients with advanced Kienbock's disease who were followed up for 2 to 16 years and then analyzed according to Evans's scoring system.</p><p><b>RESULTS</b>Aseptic necrosis did not occur in the transposed capitate because the pedicle fascia including the dorsal branch of the anterior interosseous artery ensured the vascularity of proximal two thirds of the capitate. The transposed capitate reestablished a relatively pain-free radiocarpal joint. Follow-up results showed that grip strength and motion arc were up to 70% of contralateral side.</p><p><b>CONCLUSIONS</b>Capitate transposition with a pedicle is a reliable treatment method for advanced Kienbock's disease, with favorable prognosis for at least five years postoperatively.</p>

<p><b>OBJECTIVE</b>To analyze the long-term outcomes of antebrachial flap using a pedicle of the dorsal interosseous artery.</p><p><b>METHODS</b>Ninety cases were followed up for 1-16 years, in which the indications, long-term outcomes and dorsal aspect of the forearm were assessed retrospectively.</p><p><b>RESULTS</b>Eighty-nine cases survived with satisfactory contour of the forearm and hand.</p><p><b>CONCLUSIONS</b>Antebrachial flap using a pedicle of the dorsal interosseous artery is an optimal method for repair of soft tissue defects in the hand.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the diagnosis and operative treatment of radiation-induced brachial plexopathy.</p><p><b>METHODS</b>Nine cases of radiation-induced brachial plexopathy were divided into two groups, 4 cases undergoing neurolysis of brachial plexus as Group A and 5 cases undergoing transfer of myocutaneous flaps after neurolysis as Group B. In Group B, 4 cases were treated with latissimus dorsi myocutaneous flaps (about 20 cm x 20 cm) and 1 case with pectoralis major myocutaneous flap (about 8 cm x 6 cm).</p><p><b>RESULTS</b>All the 9 cases of radiation-induced brachial plexopathy were followed up for a period of 2 to 5 years, with an average of 2.3 years. As far as pain relief and function recovery were concerned, the results of Group B were better than those of Group A.</p><p><b>CONCLUSIONS</b>Based on the results of Group B in the series, we suggest that the procedure of covering the wounds with transferred myocutaneous flaps after neurolysis of the brachial plexus should be performed to those advanced patients. The procedure may improve the blood supply of the fibrotic brachial plexus by reestablishing a good nerve bed.</p>

ABSTRACT

Objective To report the long-term outcomes of repairing finger soft tissue defects using the dorsal metacarpal flap and compund flap. Methods Through a review of 122 cases,who were treated using the dorsal metacarpal flap and compound flap,the indication and long-term outcomes of the flap and characteristics of blood supply and compound tissues transferred and venous return were analyzed. Results All cases survived unevently.The fingers which were repaired using the flap were normal,and the contours of the donor sites were normal. Conclusions The dorsal metacarpal flap and compund flap are an optimal method to repair finger soft tissue defects.
